1.Fill out the rebate application form here. Within 90 days from the date of purchase of your AED.
2. The AED must be "permanently installed in a publicly accessible location within one hundred fifty (150) feet from the entrance of the building or in another highly trafficked area of the building" and proof of installation provided.
3. There is a maximum rebate of $400 for any one AED and a maximum rebate of $750 total per single address.
4. There must be a Cardiac Emergency Response Plan (CERP) for your AED location. We can help in creating this plan.
5. You must register the AED with the AED registry here:
